Lip Filler Aftercare: 48-Hour Do's and Don'ts for Best Results

I keep lip filler appointments quick, usually about 30 minutes, but the next 48 hours are where you protect the result. The single most important rule is simple: don’t touch, press, or massage your lips unless I specifically tell you to. In New York, it’s easy to forget this when you’re rushing through Midtown East, grabbing coffee, or hopping on a crowded subway. Leave your lips alone and let the filler settle.
Quick reality check: swelling and a little asymmetry early on is common. What I care about is that swelling trends down, your lips stay comfortable, and nothing looks or feels alarming.
Your 0–48 hour lip filler checklist (the stuff that actually matters)
Right after lip filler, my goal is simple: keep swelling and bruising as low as possible while the product settles into the shape we created. For the first two days, I generally recommend these do’s and don’ts.
Do this
- Use a cold compress intermittently for swelling. Short sessions work better than parking ice on your lips for a long time.
- Keep the area clean. Gentle cleansing is fine.
- Sleep with your head elevated the first night or two. It often helps swelling settle faster.
- Stay hydrated and eat softer foods if you’re feeling tender.
Avoid this
- Exercise for at least 24 to 48 hours. Increased blood flow tends to increase swelling and bruising.
- Alcohol for at least 24 to 48 hours. It can make bruising worse for a lot of people.
- Heat like hot baths, saunas, steam rooms, or very hot showers. New York summers can already run hot, so be extra careful in August.
- Touching, pressing, picking, or massaging. If you feel a small bump, don’t try to “fix” it yourself. Text or call me.
- Makeup directly on the lips for the first 24 hours.
If you commute through Midtown East or Turtle Bay, I’m a big fan of keeping a small cold pack in your bag for the ride home, especially if it’s warm out and you’re in close subway crowds. Heat and lots of hands near faces are not your friend during the first day.
Days 3–7: how to keep lip filler looking smooth and natural
Most people feel much more normal after the first couple of days, but your lips are still settling. This is the week where patience pays off.
Swelling and symmetry: what’s normal
Mild swelling can hang around for several days, and it’s common for one side to look slightly more swollen early on. It usually evens out as swelling drops. If something looks dramatically uneven and it’s getting worse, I want you to reach out.
Heat, sweat, sun, and irritation
If you’re getting back to workouts, start light. If you’re outside in the sun, keep things gentle and avoid aggressive exfoliants around the mouth. And if you’re in a New York summer stretch, keep your lips cool and clean, especially if you’re commuting daily through crowded stations.
Lip balm, lipstick, and skincare: what I want on your lips (and what I don’t)
The first day is simple. Keep it clean, keep it calm. I usually recommend skipping lipstick for 24 hours, and then easing back in if there’s no irritation. If you use lip balm, stick with something basic and non-irritating. Strong actives and “plumping” products often sting and can make you feel more swollen than you are.
My quick rule for products
- Good: gentle cleanser, simple lip balm, cool compress, clean pillowcase.
- Pause for a week: strong acids, harsh scrubs, aggressive facial tools around the mouth.
- Avoid: plumping glosses and anything that makes your lips tingle or burn.
And if you had filler done for more than just the lips, like dermal fillers for nasolabial lines, keep the same mindset: clean hands, no rubbing, and no heat right away. Cosmetic fillers settle best when you don’t “work” them at home.
When to call me after lip filler (and when it’s just normal swelling)
I’m easy to reach, and I’d rather you check in than sit there worried and Googling. A quick message and a photo can often clear things up fast, especially if you’re coming in from the Upper East Side, Midtown, Brooklyn, Queens, or even Jersey City and Hoboken and want to know if you need a same-week check.
Call right away if you notice:
- Severe pain that doesn’t feel like normal tenderness
- Blanching or whitening of the skin, dusky discoloration, or spreading numbness
- Increasing asymmetry that’s getting worse, not better
- Fever, pus, or signs of infection
If symptoms feel urgent or severe, seek emergency care immediately.

Busy schedule? I get it. If you’re trying to keep downtime low between office hours and social plans, follow the 48-hour rules tightly, then ease back into workouts and heat slowly. That’s usually the difference between “a little puffy” and “why am I still swollen?”
A quick note on results: why aftercare helps lip filler last
Lip filler results typically last 6 to 12 months, but the first week sets the tone. Swelling management, avoiding heat, and keeping your hands off your lips all help you see the shape we intended sooner. And when you protect your result early, you’re less likely to feel like you “need more” just because swelling hid the definition.
